TENDER NO: CHAZ/GF/HRD10/ICB1/12

TENDER FOR THE SUPPLY AND DELIVERY OF LABORATORY REAGENTS (HIV TEST KITS).

1. The Global Fund for HIV/AIDS, TB and Malaria has released funds to the Churches Health Association of Zambia and it intends to apply part of the proceeds to payments under the contract for the supply and delivery of LABORATORY REAGENTS (HIV TEST KITS). The tender will be processed on International Competitive Bidding (ICB) procedure.

2. The Churches Health Association of Zambia now invites sealed bids from eligible and interested bidders for the supply and delivery of LABORATORY REAGENTS (HIV TEST KITS).

3. Bidders may bid for a single item, any number of items or all of the items. CHAZ will evaluate the bids for each item and award the contract(s) on item by item basis accordingly. Bidders not quoting for all required quantities for an item will be considered non-responsive.

4. A complete set of bidding documents may be purchased from the Churches Health Association of Zambia , upon payment of a non-refundable fee of K 500,000.00 or its equivalent in any freely convertible currency at the prevailing exchange rate, in cash or Bank certified cheques.

5. Bids should be sent clearly marked “Tender for the supply and delivery of laboratory reagents (HIV Test Kits) and addressed to: Executive Director, Churches Health Association of Zambia , Plot No. 9306 Ben Bella Road P.O Box 34511 Lusaka Zambia. Tel: 260-211-237327/8/229702/239946 Fax: 223297/239946, HOWEVER, TELEGRAPHIC AND/OR ELECTRONIC OFFERS WILL NOT BE ACCEPTED.

6. Bids must be deposited in the Tender Box at the Churches Health Association of Zambia on Plot No. 9306 Ben Bella Road, Lusaka, Zambia on or before Friday 9th March, 2012 at 14:30 hours local time.

7. The closing date for the receipt of bids is Friday 9th March 2012 at 14:30 hours local time and any bids received after the time and date stipulated above will not be accepted. The bids will be opened immediately thereafter in the Conference Room, in the presence of bidders or their representatives who choose to attend.
